Here's how to get started and what you'll need to bring.
Your first appointment (Initial Evaluation)
To get started give us a call/text us at (307) 773-8533 or visit our office. We treat a variety of injuries and conditions including: athletic injuries, back pain, post-operative rehabilitation, postural dysfunctions, trigger points, preventative medicine, general conditioning, just to name a few. Please arrive 30 minutes early for your first visit, and expect the initial visit to last 45-60 minutes. We recommend wearing comfortable, loose-fitting clothes so your therapist can appropriately assess the area of pain or injury. Don't worry, we've got changing facilities at our locations. Don’t forget your prescription! Most but not all insurance requires a prescription in order to be treated in physical therapy. Wyoming is a direct access state in which you do not need a prescription to be treated by a physical or occupational therapist; however your insurance may require a prescription. Depending on your individual situation, you may be able to request a prescription by phone with your medical provider, or you may be required to complete an office visit prior to receiving a therapy ‘script’. Paperwork By printing and filling out our Intake Form before your first appointment, you can help save time in getting started with your initial evaluation. If you are unable to print these out we can provide them on site. Also select one of the forms to fill out regarding the area of your complaint: Subsequent Appointments Please try to arrive at least 5 min early and check in with the front desk so your therapist knows you have arrived. If you can't make it to your appointment for any reason contact us ASAP to reschedule or cancel. |
Insurance & Payment
Give us a call and we will let you know what insurances we accept. As a courtesy, we contact your insurance company during the first visit. Keep in mind, though, that we cannot guarantee the verbal quotes that we receive. For the best estimate of your benefits, we recommend that you also contact your insurance company directly to inquire about coverage for physical therapy in an office setting. You may also refer to your written insurance plan summary for assistance. Payment. Co-payments and/or co-percentages are collected at each physical and occupational therapy visit. We accept cash, checks, or credit card payments. We Accept: Visa, Mastercard, Discover Private and Cash Pay options For Patients with no physical referral, no insurance coverage, not billed to insurance and due at time of service
